It is simple to start the enrollment process. Please fill out the Camper Intake form. Once the form is received, Anna Cozzi, our Program Director, will contact you.
NEW CAMPERS What you can expect from enrollment process:
- Fill out Intake Form.
- If your camper seems like an appropriate fit for camp, Anna Cozzi will contact you.
- Fill out the 2nd Intake Form.
- Submit 3 Release of Information forms so we may contact the professionals working with your camper.
- Schedule Intake Interview.
- Once we have conducted the intake interview and contacted all professional references, we will be able to extend an acceptance to Camp Altitude!
How long do we wait before we know about acceptance?
The enrollment process is very smooth. The length of time will be determined by how long it takes to submit all forms (Intake Form 1, Intake Form 2, and 3 Releases), schedule and conduct the interview, and how soon we can contact professionals. In order to expedite the enrollment process, please let all professional references know that Camp Altitude will be calling. The whole enrollment process can take place in a week.
RETURNING CAMPERS
Thank you for coming back to Camp Altitude for another summer… we love our returning campers! We do know that all children grow and change within the year, and this is why it is so important to get an update about your camper before camp starts.
1) Fill out Returning Camper Intake
2) Anna Cozzi, Program Director, will contact you to discuss any changes or updates with your camper to update their file.
3) New information will be reviewed with clinical staff, then an acceptance will be extended.
WEEKEND ALTITUDE ENROLLMENT
Weekend Altitude: Family Camp also has open enrollment for our weekend camps offered throughout the year.
Steps for enrollment:
1) Fill out Intake Form and indicate what weekend you are interested in
2) Fill out Registration form
3) Anna Cozzi, Program Director, will contact you to let you know all documents have been received.
